Anyone interested in swinging a bat or throwing a ball at Madtown Sluggers must fill out one of the waivers below.  It's easy, first, find out if you are a minor or an adult, click on the appropriate form, fill in the form and bring it with you when you come to train.  For further convenience, you can email it to madtownsluggers@gmail.com if you would like.  Sorry for the inconvenience, but we have to cover our butt.
